Room Reservations

MOSAIC offers two types of rooms available for reservation. The front lounge area can accommodate up to 40 people, with flexible setup. The back conference room is typically used for small group meetings of six people.

If you would like to host an event at MOSAIC, please complete both the Room Reservation Request and the Facility Use Contract. Both forms must be turned into a MOSAIC staff member at least one week prior to the event. All events should be congruent with MOSAIC's mission.

Facility Use Contract

The requester/authorized agent must adhere to the conditions of the contract. Please print, read and sign our Facility Use Contract . Note: Adobe Acrobat Reader is required to view the forms. If you do not have Acrobat Reader, you can download it for free from http://www.adobe.com/.
